Tony is a Certified Physician Assistant who studied Health Promotion and Education in his undergrad, then completed his Master’s Degree in Physician Assistant Studies at the University of Utah. Tony is currently a Board Certified Physician Assistant. Tony loves all aspects and phases of family medicine. Tony speaks English, Portuguese and Spanish. Tony enjoys helping people establish and achieve their optimum health. Personally, Tony enjoys outdoor activities and spending time with his family.

Katie received her Doctorate of Nursing Practice from the University of Wisconsin Milwaukee. She is a Certified Family Nurse Practitioner. Katie is passionate about preventative health and enjoys developing relationships with patients and families while working with them to achieve their health-related goals. Katie speaks English and Spanish. In her free time, Katie enjoys traveling and exploring the outdoors with her husband and dog.
Nadia is a Certified Physician Assistant who studied nutrition in her undergrad, then completed her Master’s Degree in Physician Assistant Studies at Touro University Nevada. Nadia speaks English and Spanish. Nadia works at Mid-Valley Health Clinic two days a week and is found at our Mobile Medical Clinic Tuesday & Wednesday each week.

Veronica is a Board Certified Physician Assistant who studied Health Promotion and Education in her undergrad and completed her Master's of Physician Assistant Studies at the University of Utah. Veronica enjoys the wide spectrum of medicine that comes with practicing in primary care. She focuses on patient centered care that incorporates prevention and management. She speaks English and Spanish. She is a Utah native that enjoys the outdoors.
Dr. Rodriguez graduated from Utah Valley University Magna Cum Laude in Biology. He graduated from Roseman College of Dental Medicine with a Doctor of Dental Medicine (DMD) and Masters of Business Administration (MBA) degree. During dental school Dr. Rodriguez was the recipient of the Implant Dental Award, Distinguished Student Award and Excellence in Prosthodontics Award. He was also a recipient of the four year National Health Service Corps (NHSC) Award. After graduating Dr. Rodriguez did a one year Advanced Education in General Dentistry (AEGD) residency through New York University Langone. There he gained more experience and became proficient in difficult surgical extractions, taking out wisdom teeth, rehabilitating complex partial and full mouth cases, root canals and dental implants.
Melissa received her Bachelors of Applied Science with a Major in Dental Hygiene from Weber State University in April 2019. She has worked throughout the Salt Lake Valley as a Hygienist and aided in increasing the understanding of the importance of dental health in her community. Melissa is passionate about her career and loves interacting with her patients. She believes dental health is a very important part of overall physical health. Melissa is fluent in English and Spanish. When Melissa is not working, she enjoys spending time with her family and taking her dogs for a walk.
Dr. Rodriguez graduated from Utah Valley University Magna Cum Laude in Aviation Science. She graduated from Roseman College of Dental Medicine with a Doctor of Dental Medicine (DMD). During dental school Dr. Rodriguez was the recipient of the distinguished Student Award of Excellence in Patient Care. She was also a recipient of the four-year National Health Service Corps (NHSC) Award. Her husband, Daniel, is also a dentist and Dr. Rodirguez loves that she can collaborate with him and share the same professional passion as her husband. After work, Dr. Rodriguez enjoys spending time with her husband, six children, and two poodles hiking, watching movies, or playing board games.
Leslie is a Registered Dental Hygienist who graduated from Salt Lake Community College with an Associate of Applied Science in Dental Hygiene and an Associate of Science in Health Sciences. Leslie speaks English and Spanish. Outside of work, she enjoys theatre, reading, and hiking.

Phil is a Certified Physician Assistant who studied Biology at the University of Utah during his undergrad, then completed his Master’s Degree in Physician Assistant Studies at Indiana State University. He speaks English and Spanish. He enjoys the pivotal role of primary care in helping patients optimize their health. Outside of work, he enjoys board gaming, and spending time with his family traveling.
Doctor of Medicine
Dr. Nathan Zacharias, MD has 5 years of experience doing primary care, urgent care, taking care of hospitalized patients, and doing telemedicine. He is Board-Certified in both Family Medicine and Hospice & Palliative Medicine. When he isn't working, he's rock climbing, trail running, skiing, or finding some other excuse to be in the mountains. He speaks both English and Spanish.
Licensed Clinical Social Worker
Abigail received her Master’s degree in Social Work at Utah State University. She has an undergraduate degree in Law from BUAP, Mexico. Abigail likes to care for all ages, but especially enjoys working with children. She specializes in treating patients who have experienced domestic abuse, depression, anxiety, and trauma. Abigail enjoys the outdoors, hiking, swimming, and especially vacations on the beach. Abigail is married with three children.
Associate Clinical Mental Health Counselor
Alexis graduated with her Master's Degree in Clinical Mental Health Counseling in 2023 and recieved her undergrad degree in Psychology and Human Development from The University of Utah (go utes!). Alexis works primarily with adults and enjoys working with clients of all backgrounds. Alexis loves working with an integrated care model to help eliminate any gaps in care. In her free time she enjoys reading, traveling, and spending time with her husband and daughter.
Dr. Farrell is a graduate of the Southern California College of Optometry and has been a practicing optometrist since 1979. Dr. Farrell is fluent in Spanish. He enjoys working with all ages, but especially enjoys providing corrective eye glasses to children. “It is a absolutely wonderful to see children’s faces light up when they can see clearly for the first time”.
Jack Wang received his undergraduate
B.A. in Biology from The University of Texas at
Austin. He then pursued his optometry
degree from the University of Houston College
of Optometry. His main focus is practicing primary eye care. But he also likes traveling around the mobile eye clinic to provide vision exam to kids. Outside of clinic, he enjoys spending time with his family and dogs. Other hobbies include cooking and keeping up with current events.
Heidi Schiller, PharmD, MBA was born and raised in Utah. She has a passion for providing the best care and pricing for our undeserved population in West Jordan and the surrounding areas. She has been in practice for 2 years and graduated from the Roseman University of Health Sciences, College of Pharmacy and Master of Business Administration in 2021. Upon graduation, she began her career opening a pharmacy for Utah Partners for Health as the pharmacist-in-charge. While here, she became proficient in immunizations, pharmacy billing, 340B medication, and pharmacy management. She administers immunizations and dedicates her time to helping patients better understand their medications and side effects. Heidi believes that 340B pharmacies play a vital role in the healthcare of patients by providing medication pricing they would not be able to obtain anywhere else. Besides managing Utah Partners for Health pharmacy, she has opened two other independent pharmacies in Utah. Personally, she enjoys hiking, camping, and spending time with her daughter Raven, son Xander, and husband Max.
Venkata Reedy Gudipati, PharmD, was raised in India, he moved to the United States 23 years ago to pursue his pharmacy degree. He has been in practice for 20 Years and graduated with a Doctor of Pharmacy degree from HKES School of Pharmacy in 1996. After graduating with his pharmacy degree, he opened a retail pharmacy in The Villages, Florida where he dedicated his time to providing pharmaceutical care to the residents of Florida. While running his pharmacy he also worked as a pharmacist at the University of Florida Hospital for 6 years. In 2013 Venkata sold his pharmacy and shortly after moved to Utah to pursue his career as a retail pharmacist and raise his family. He worked as a retail pharmacist with Walgreens for 18 years before joining Utah Partners for Health in September 2021. Since joining Utah Partners
for Health Venkata has also opened two independent pharmacies in Utah. In his spare time, he enjoys traveling with his wife and their two sons.
