Our Promise

We are a patient-centered family medical home (PCMH) which means our providers can provide primary care services to patients of ALL AGES.  We network with many specialists, and additional service providers, to connect you with quality specialty care at reduced prices when needed. As a patient, you are encouraged to establish a relationship with your provider, and to see the same team for each visit, so they can really get to know you and your medical needs.

Patient Rights

At UPFH we value and respect you.  We offer every patient the same access to care, the same rights, and the same responsibilities, regardless of race, color, culture, language, ethnicity, religion, gender, sexual orientation, socioeconomic status, age, and physical or mental disability. Our goal is to provide excellent health care to empower you and your family to work together with us to improve your health and overall well-being and the well-being of our community. 

It is your responsibility to: 

  • Give correct and complete information about your health status and health history. 

  • Ask questions if you do not understand information or instructions. 

  • Cooperate with your caregivers. 

  • Respect the rights and property of other patients. 

  • Tell your caregivers of any medications you brought from home. 

  • Report any changes in your health status to your caregivers.

  • Proper evaluation and treatment 

  • Have your concerns heard and resolved when possible 

  • Be informed about your health status, recommended treatments, options, risks and benefits. 

  • Make decisions about your treatment 

  • Be involved with and informed about your care through discussions with your providers.
