What is Income Verification?
UPFH is not a free clinic; we’re the best clinic and we can save you the most money on your office visit payments in the valley when you provide income verification annually. You can get the discount even if you have insurance!
Verifying your income means providing proof of what you earn. You will then be on a Sliding Scale Discount Fee Schedule depending on 1) your family income and 2) your family size. Some discounts require proof of address as well.
