Tony studied Health Promotion and Education before completing his Physician Assistant Studies at the University of Utah. He loves all aspects and phases of family medicine. Fluent in English, Portuguese and Spanish, Tony enjoys helping people establish and achieve their optimum health.

Katie received her Doctorate of Nursing Practice from the University of Wisconsin Milwaukee. Katie is passionate about preventative health and enjoys developing relationships with patients and families while working with them to achieve their health-related goals. Katie speaks English and Spanish. In her free time, Katie enjoys traveling and exploring.
Veronica Kitchens, PA-C, MPAS

Veronica studied Health Promotion and Education in her undergrad and completed her Master's of Physician Assistant Studies at the University of Utah. Veronica enjoys the wide spectrum of medicine that comes with practicing in primary care. She focuses on patient centered care that incorporates prevention and management. She speaks English and Spanish.
Dr. Nathan Zacharias, MD
Yellow Team
Since 2016, Dr. Nathan Zacharias has focused on primary care, urgent care, taking care of hospitalized patients, and telemedicine. He is Board-Certified in both Family Medicine and Hospice & Palliative Medicine. When he isn't working, he's rock climbing, trail running, skiing, or finding some other excuse to be in the mountains. He speaks English and Spanish.
Philip Swink, PA-C, MPAS

Phil studied Biology at the University of Utah during his undergrad, then completed his Master’s Degree in Physician Assistant Studies at Indiana State University. An English and Spanish. speaker, he enjoys the pivotal role of primary care in helping patients optimize their health. Outside of work, he enjoys board gaming, and spending time with his family traveling.
Nadia Xibille, PA-C, MPAS

Nadia studied nutrition in her undergrad, then completed her Master’s Degree in Physician Assistant Studies at Touro University Nevada. A native Spanish-speaker fluent in English, Nadia works at Mid-Valley Health Clinic two days a week and is found at our Mobile Medical Clinic Tuesday & Wednesday each week.
Dr. Sarah Woolsey, MD
Sarah is a Board-Certified family physician that trained in Chicago at the University of Illinois and completed her residency at the University of Utah. She speaks English and Spanish and is happy to see patients in person or through telehealth. She has 25 years experience as a community health doctor. In her spare time she loves hiking, road biking, and cooking.
Abigail Gonzales,
BH Director & Licensed Clinical Social Worker
Abigail received her Master’s degree in Social Work from Utah State University after graduating in law from BUAP, Mexico. Abigail likes to care for all ages, but especially enjoys working with children. She specializes in treating patients who have experienced domestic abuse, depression, anxiety, and trauma. Abigail enjoys the outdoors, hiking, swimming, & vacations on the beach. Abigail is married with three children.
Alexis Anthon, Mental Health Counselor
​Alexis graduated with her Master's in Clinical Mental Health Counseling in 2023 after getting a degree in Psychology and Human Development from the U of U (Go Utes!). Alexis works primarily with adults and enjoys working with clients of all backgrounds. Alexis loves working with an integrated care model to help eliminate gaps in care. In her free time she enjoys reading, traveling, and spending time with her husband & daughter.
Nancy Raymond, FNP-C
Nancy received her Master’s degree as a Family Nurse Practitioner from Chamberlin University after getting a Bachelor's as a Registered Nurse, BSN. Since 2021, Nancy has focused her career on mental health and medication management. She recently completed her PMHNP, which allows her to take her Boards. Nancy enjoys cats, reading & spending time with her family.
Dr. Jack Wang Optometrist
Jack Wang received his undergraduate
B.A. in Biology from The University of Texas at
Austin. He then pursued his optometry
degree from the University of Houston College
of Optometry. His main focus is practicing primary eye care. But he also likes traveling around the mobile eye clinic to provide vision exam to kids. Outside of clinic, he enjoys spending time with his family and dogs. Other hobbies include cooking and keeping up with current events.
Dr. Scott Farrell Optometrist
Dr. Farrell is a graduate of the Southern California College of Optometry and has been a practicing optometrist since 1979. Dr. Farrell is fluent in Spanish. He enjoys working with all ages, but especially enjoys providing corrective eye glasses to children. “It is a absolutely wonderful to see children’s faces light up when they can see clearly for the first time”.
Dr. Daniel Rodriguez, Dentist
After graduating from UVU Magna Cum Laude in Biology, Dr. Rodriguez received Doctorate of Dental Medicine (DMD) and MBA degrees from Roseman College where he was awarded the Implant Dental, Distinguished Student, Excellence in Prosthodontics, and the four-year National Health Service Corps (NHSC) Awards. During residency at New York University Langone, Dr. Rodriguez became proficient in difficult surgical extractions, wisdom teeth removals, root canals, dental implants, and rehabilitating complex partial & full mouth cases.
Dr. Savanah Rodriguez, Dentist
Hailing from UVU--Magna Cum Laude in Aviation Science-- Dr. Savanah Rodriguez got a Doctor of Dental Medicine (DMD) at Roseman College, receiving the Student Award of Excellence in Patient Care and the four-year National Health Service Corps (NHSC) Award. Her husband, Daniel, is also a dentist and Dr. Savannah loves collaborating and sharing the same professional passion. After work, Dr. Rodriguez enjoys spending time with her husband, six children & two poodles, hiking, watching movies, or playing board games.
Heidi Schiller, Pharmacist
Passionate about providing the best care and pricing, Heidi opened UPFH Pharmacy as the lead pharmacist in 2021 as soon as she graduated with a pharmacy degree and MBA from Roseman University. She's dedicated to helping patients better understand their medications & side effects. Heidi believes 340B pharmacies play a vital role in serving those in need and has opened two other independent pharmacies. She enjoys hiking, camping, and spending time with her daughter Raven, son Xander, and husband Max.
Venkat Reddy-Gudipati, Pharmacist
Raised in India, Venkata Reedy Gudipati moved to the U.S. 23 years ago to pursue his pharmacy degree. In 1996, he earned his Doctor of Pharmacy from HKES School of Pharmacy and opened a retail pharmacy in Florida. eventually also working for the University of Florida Hospital. Venkata sold his pharmacy in 2013 and moved to Utah to raise his family, joining UPFH in 2021 and opening two other independent pharmacies. In his spare time, he enjoys traveling with his wife and their two sons.
