Utah Partners for Health (UPFH) exists because people matter!
Our mission is to be everyone’s first choice in medical care as we provide compassionate, comprehensive, quality care to empower underserved individuals, families, and communities in a patient centered medical home.
Our Promise
We are a patient-centered family medical home (PCMH) which means our providers can provide primary care services to patients of ALL AGES. We network with many specialists, and additional service providers, to connect you with quality specialty care at reduced prices when needed. As a patient, you are encouraged to establish a relationship with your provider, and to see the same team for each visit, so they can really get to know you and your medical needs.
