Low Income? Get Discounts for Doctor Visits

What is a Sliding Fee Discount?


Sliding Fee Discounts are fees for services that are adjusted depending on an individual's income and family size. UPFH sets these discounts for our patients to allow for fairness and to address income inequality. The higher your income, the more you will pay; the lower your income, the less you will pay.


Example of Sliding Fee Discount Schedule



In this example, if an individual in Slide A* visits a UPFH medical provider, that person will pay $35 for the appointment, even if they have no insurance. If that individual has insurance, UPFH will charge the insurance, but the individual will still only pay $35 out of pocket, even if the insurance co-pay is higher than $35.


Let's do another example.



If a person in Slide B* has a counseling appointment, they will pay no more than $25 for that appointment, whether or not they have insurance.


Remember, all health insurance has to be reported to UPFH! That's a government regulation. Health insurance companies need to do their part. Plus, UPFH tells your insurance when you've paid something that counts for your deductible so you never pay more than you have to. As a community health center, UPFH takes the amount your insurance pays and donate all proceeds to medical care for those in need. Without the support of ALL our community, those in need cannot get care!


What is My Discount Slide?


You will be in Slide A, B, C, D, E or F depending on 1) your family income and 2) your family size, after you 3) verify your income. This means providing proof of what you earn. Some discounts require proof of address as well.


To find out what Slide YOU are on and what YOU pay for an appointment, see our helpful Front Desk. You can ask for a printed estimate of your visit payment, called a Good Faith Estimate. It is only an estimate. If, for example, you see your provider and need an x-ray during your appointment, the x-ray service may be an additional charge. You can ask about price before any procedure.
